The first cancer-directed therapy (treatment given in an attempt to destroy or modify cancer tissue) was surgery.
Excisional biopsy. A single procedure was done (stated to be a wide excision), but the margin is less than 1 centimeter. If the excision does not have microscopically negative margins greater than 1 cm, use the appropriate code 20-36.
Code the most extensive surgery if the patient has multiple surgical procedures of the primary site.
