The grading of urothelial (transitional) tumors is based on the WHO Classification, which is a two-grade system. These histologies are either low-grade (L) or high-grade (H). Although the low-grade and high-grade are part of the histology name, the low and high can be used for assigning grade.
For the non-transitional histologies (i.e., squamous cell carcinoma, adenocarcinoma), these use a three-grade system. This three-grade system is based on the nuclear grading system.

- Moderately differentiated
- Poorly differentiated, undifferentiated, anaplastic
Grade is to be coded from the original tumor. Do not use grade information from lymph nodes, metastatic tissue, or recurrences.
